Use of up-to-date laboratory methods for timely and reliable detection of tuberculosis patients positive for M.tuberculosis allows early determination of patient category by clinicians, prescription of adequate chemotherapy regimens, shortening of time to sputum conversion, and as a result, improve treatment effectiveness and prevent the spread of drug-resistant strains of M.tuberculosis. We comparatively analyzed results of M.tuberculosis detection using single specimen subjected to the following tests: molecular genetic GeneXpert PCR analysis, fluorescence microscopy, Bactec-960 test system, and traditional culture test on solid media. Specimens from 154 patients were analyzed. Based on test results, it was concluded that a timely detection of infectious tuberculosis patients requires making use of all battery of available bacteriological methods

To assess the effectiveness microscopic methods used in the Phthisiatry Research-Practice Center, we comparatively studied the results of Ziehl-Neelsen microscopy, fluorescence microscopy and LED-microscopy in detecting acid-fast bacilli (AFB).We comparatively analyzed (using Ziehl-Neelsen microscopy) a total of 400 positive smears with AFB detected by fluorescence microscopy, and specimens from 648 patients treated at Pulmonology department of the Yakutsk City Clinical Hospital, who were tested by their clinical diagnostic laboratory as negative for AFB after triple Ziehl-Neelsen microscopy of intact sputum. Analysis of trends in statistics for smear-positive patients (MTB+) over the period from 2011 to 2016 showed that number of registered MTB+ patients with active pulmonary tuberculosis had decreased during the study period, both among all cases, and among new cases. Nonetheless, the size of infection reservoir defined as the number of MTB+ patients remains at high levels. We observed high rates of prevalence for drug-resistance, including multidrug resistance and extensive drug resistance (MDR and XDR). Prevalence rates of tuberculosis cases with MDR in Yakutia in the years 2013 to 2015 were higher than the same rates over Russian Federation by factors of 1.7-1.8-1.6, but showed a trend to decrease compared to the rates in the Far East Federal District. High proportion of newly identified patients with MDR among patients with drug-resistant tuberculosis (up to 68.5%) suggests the emergence of a totally new-made tuberculosis infection reservoir with resistance to first-line drugs. The policy pursued and supported in the Sakha Republic (Yakutia) is aimed at strengthening laboratory capabilities, adopting novel diagnostic technologies, improving epidemiologic surveillance, while supporting the currently running state programs against tuberculosis, and considering the currently observed decrease in tuberculosis incidence, this could lead to beneficial effect on existing and future burden of drug-resistant tuberculosis
