Suicide prevention is a topical issue in modern healthcare. There are northern regions in the Russian Federation where the suicide rate exceeds Russian indicators by 2.8-3 times. The purpose of this study is a comparative analysis of the suicidal index (SI) in the Arctic zone of the Republic of Sakha (Yakutia) and among American Indians - residents of Alaska, to identify the main factor contributing to the widespread suicidal behavior among residents of the Far North. Materials and methods of research. A comparative analysis of the materials of the organizational and methodological department of the State Budgetary Institution of the RS(Ya) “Yakut Neuropsychiatric Dispensary”, as well as data from the territorial body of the Federal State Statistics Service for the Republic of Sakha (Yakutia) on the mortality of the population of the Arctic zone of the RS(Y) from suicide and statistical data on suicides of the Center for Disease Prevention and Control of the USA - CDC (Centers for Disease Control and Prevention) for 2018 and 2021 . It was found that from 2018 to 2021, the SI increased among residents of the Arctic zone of the Republic of Sakha (Yakutia) by 34 %. For American Indians living in Alaska, the SI increased by 23 % over the same period of time. The SI for 2018 for residents of the Arctic zone of the Republic of Yakutia is 2.6 times higher than for residents of the Russian Federation. The suicide index (SI) for 2018 among American Indians - residents of Alaska is 1.6 times higher than that of residents of the United States. The SI for 2021 for residents of the Arctic zone, the RS(Ya) is 4 times higher than for residents of the Russian Federation. The SI for 2021 the American Indian population of Alaska is 2 times higher than that of the residents of the United States. It was established that there are two regions in the world with extremely high suicide rates - Greenland and Chukotka Autonomous District in the Russian Federation. Both regions differ from other parts of the world in that they are located at the highest geographical latitudes. These are territories with the most extreme living conditions. The Arctic zone of the RS(Ya) belongs to the coldest regions of the Russian Federation. Remoteness from the central regions and extreme living and working conditions lead to a shortage of personnel in the Arctic zone of the RS(Ya) of psychiatrists. Consequently, the extremely high suicide rate in the Arctic zone of the RS(Ya) is influenced by the difficulty of accessing necessary psychiatric care and the lack of preventive mental health work.

The article describes a case of the development of a severe form of bronchopulmonary dysplasia in a premature newborn. Globally, about 15 million children are born prematurely every year. On average, it is every tenth child. In Russia, more than 110 thousand children are born prematurely every year, which is about 5.7 % of all newborns per year. [8] This confirms the relevance of the chosen topic. The ABS test falls under the category of express studies, since its parameters change rapidly with any shifts in the patient’s condition (respiratory indicators, body temperature, physical activity, kidney function, etc.). [9] The study of blood pH revealed subcompensated metabolic acidosis, confirming the literature data. The gas composition of the blood varied from hypocapnia to hypercapnia, which indicated a severe degree of respiratory failure with a picture of hypoventilation of the lungs in premature newborns. The revealed decrease in electrolytes was associated with diuretic and steroid therapy used for the patients with bronchopulmonary dysplasia. Due to prematurity, compensatory mechanisms were insufficient to maintain homeostatic parameters (pH, gas and electrolyte composition of blood). When conducting oxygen therapy with a ventilator, it is necessary to be guided by the dynamics of ABS tests to calibrate the parameters of the device and adjust infusion therapy.

The results of ischemic stroke thrombolytic therapy in the primary vascular department in Yakutia in 2019-2020 were studied. The proportion of ischemic stroke patients who received intravenous thrombolytic therapy (IVT) was 6.3% (n = 9). We investigated groups of patients with and without IVT (n = 9 and n = 62, respectively), comparable in age and sex. One third of patients had the cardioembolic ischemic stroke. 80.2% of intravenous thrombolytic therapy’s contraindications were associated with hospitalization beyond the 4.5-hour “therapeutic window”. There were no fatal outcome or symptomatic hemorrhagic complications in the IVT group. At the end of the acute period, the neurological status according to the NIHSS scale was comparable (p = 0.488), as was the degree of functional independence according to the Rankin Scale (p = 0.819) between groups with and without intravenous thrombolytic therapy. In the Yakutia, taking into account the low population density, large territory, complex transport scheme and peculiarities of climatic conditions, it is advisable to use the possibilities of telemedicine for conducting thrombolytic therapy of ischemic stroke. In the republic, for the increasing the account of intravenous thrombolytic therapy it is necessary to introduct the telethrombolysis method in regional hospitals that have computed tomography devices, as well as in primary vascular departments for the providing teleconsulting with the neuroimaging analysis in difficult clinical cases. The creation of a telethrombolysis network will increase the availability of modern methods of specialized medical care for patients with acute cerebrovascular accidents in areas with vast territories and low population density

Parkinson’s disease is one of the most common neurodegenerative disease and found in all countries of the world. The average prevalence of the disease varies from 120 to 180 per 100 thousand population. Parkinson’s disease is disease of the elderly: if up to 45 years, it almost does not occur, then after 60 years of Parkinson’s disease suffers 1-2% of the population, and after 85 years - 3-5%. According to the latest WHO data, in the world of Parkinson’s disease affects more than 4 million people. This indicator will continue to grow and by 2030 will amount to 9 million people. The disease was first described in 1817 by the English surgeon James Parkinson in his “Essay on the Shaking Palsy”. However, until recently, in spite of a breakthrough science are not fully clarified the etiology and pathogenesis of the disease. Parkinson’s disease currently is considered as multifactorial disease and thus, it is intended multifactorial causes, including genetic and environmental factors. However, significant factors affecting the development of the disease is not currently identified. In recent years, widely it begins to studyof impact bowel microbiocenosis on the development of neurodegenerative diseases, including and Parkinson’s disease. The article describes the latest data of the scientific literature on the contribution of genetic, demographic and environmental factors in the development of Parkinson’s disease and discusses the current pathogenic mechanisms

The diagnostics of chronic inflammatory demyelinating polyneuropathy (CIDP) is a difficult task, as the classical diagnostic criteria are not specific enough, the clinical picture of the disease is polymorphic, and there are no reliable biomarkers of this disease. The diagnosis is based on a characteristic clinical picture and is confirmed by the detection of the demyelinating type of peripheral nerve damage in electroneuromyography or nerve biopsy, as well as excluding other diseases that may be accompanied by a similar clinical picture. Currently, the importance of lumbar puncture in the diagnostics of CIDP is being questioned, since not all patients, especially at the outpatient level, are undergoing this procedure, and typical protein-cell dissociation in approximately 40 % of patients may not be detected, especially in the first 2 weeks from the moment of the disease. Therefore, in the diagnostics of CIDP, especially its atypical forms, they introduce such methods as computerized pallestesiometry, ultrasound examination of peripheral vessels, magnetic resonance imaging of the nerve roots, and blood testing for antibodies to viruses tropic to nervous tissue. The article deals with the clinical case of a CIDP patient, who does not have a typical protein-cell dissociation in CSF, however, other paraclinic data, characteristic of inflammatory polyneuropathies, are manifested

The ability to conduct the lumbar puncture is one of the most important practical skills in Neurology. The lumbar puncture is used widely for diagnostic and therapeutic objectives. In addition to the technical difficulties, young physicians often face the psychological problems related to the uncertainty and fear of the procedure failing. Interactive teaching methods are an important step in the physician training. Lecturers at the Department of Neurology and Psychiatry hold the lumbar puncture training classes in the Simulation Center of the Medical Institute, M.K. Ammosov North-Eastern Federal University. The training is conducted on the lumbar puncture simulator which can provide the visual and tactile sensations identical to those obtained in clinical practice. The classes are held with clinical residents, interns, students in the classroom of basic practical skills. The practice consists of three stages: introduction, practical part and discussion. In the practical part the lecturer demonstrates the lumbar puncture technique on the simulator. Then the student conducts manipulation under the lecturer’s supervision. During the manipulation, the lecturer points out errors to the student. In case of an unsuccessful attempt, the procedure must be repeated. The lumbar puncture skill is counted if the procedure has been made by the student himself. In the third part of the practice, the group led by the lecturer discusses the practical work, analyzes the errors and debates the future actions. Thus, the interactive learning is a productive method in the higher medical education system. The simulator training skills improve the professional competence of the physician and it is necessary for the further clinical activities

The problem of autoimmune diseases is relatively young, about 50 years, but despite the short period, it has become the most relevant in modern clinical neurology. A special place among the demyelinating autoimmune disease is neuromyelitis optica. Neuromyelitis optica the second most frequent form of inflammatory, autoimmune, demyelinating disease after multiple sclerosis. In the last decade, the scientific understanding of Devic’s disease has expanded significantly. Neuromyelitis optica refers to rare, demyelinating diseases of the nervous system, while in the literature there is no precise data on the incidence and prevalence of the disease. To date, information about epidemiology is based on the results of individual studies. It is known, that the disease is more often diagnosed among people of Asian descent, in all populations women are more often ill. Neuromyelitis optica, unlike other demyelinating diseases, has a severe progressive course and quickly leads to persistent disability. To date, with the isolation of a specific marker (NMO-IgG), there are opportunities for an accurate diagnosis. This article presents a case of a recurring course of opticoneuromyelitis meeting the known diagnostic criteria, including the determination of aquaporin-4 antibodies of aquaporin 4 in serum. Timely diagnosis of the disease, taking into account some clinical features, using immunological methods (determination of oligoclonal antibodies, determination of the level of aquaporin-4 antibodies), instrumental methods (especially MRI in neuromyelitis optica), timely adequate treatment, will help reduce the likelihood of irreversible neurological deficit

Parkinson’s disease and essential tremor are the most common neurodegenerative diseases, which are accompanied by the development of tremor - involuntary rhythmic hyperkinesis. In the essential tremor,tremor has a kinetically postural character with a frequency of 6 to 10 Hz, affectingarms, head, face, vocal cords, tongue, legs and trunk. Parkinson tremor has a frequency of 4-6 Hz, occurring at rest in the form of “counting coins” or “rolling pills”, decreasing with active movements until complete disappearance, and is characterized by resumption after 2 or more seconds after giving the pose (“renewed” tremor). The generation of tremors is associated with dysfunction of two main ways: basal nuclei - cerebellum - thalamus and cogwheel - olive complex. Consequently, damage to any of these levels leads to the development of tremors. Recently, the concept of a single continuum of essential tremor - Parkinson’s disease - is widely discussed.It has been established that these neurological disorders can coexist or transit into each other. So, essential tremor increases the risk of developing Parkinson’s disease by 3.5 times, and in patients with Parkinson’s disease the likelihood of attaching an essential tremor is as high. This is due to the cross pathophysiological mechanism, which is probably due to genetic mutations. This article discusses the pathophysiological mechanisms of the development of tremor in patients with Parkinson’s disease and essential tremor. We describe a patient with the essential tremor whose disease has been transformed into tremor predominant Parkinson’s disease
