A long-term clinical observation of two monozygotic twin brothers with chronic hepatitis was held. Traditional general clinical laboratory tests and ELISA markers of hepatitis were used. The disease in unvaccinated twins occurred with an interval of 7 years on the background of close home contact. In both brothers acute hepatitis transformed into the chronic form, with the second one - in the shortest terms. Acute hepatitis in the second brother coincides with the peak of the exacerbation of hepatitis in the first. In this regard, vaccination of persons at risk in outbreak of viral HBV-infection is very important, as well as studying of the genetic properties of the viral agent and the response of microorganism by the help of molecular-genetic methods

The article is devoted to the epidemiology and specific prevention of hepatitis A in the Republic of Sakha (Yakutia). It presents the data on the incidence of hepatitis A for the period 2000-2017. It is shown that vaccination against hepatitis A is the most effective strategy to control this infection in modern conditions. The aim of this work was to assess the epidemiological situation with hepatitis A in the Republic of Sakha (Yakutia). Materials and methods. The paper uses the materials of official statistics of the Territorial administration of Rospotrebnadzor of the Republic of Sakha (Yakutia) and the data of the Department for the treatment of patients with viral hepatitis SBU RS (I) “Yakutsk City Clinical Hospital”. Conclusions and results. As a result of selective immunization of risk groups over the past decades, the republic has managed to reduce the incidence of viral hepatitis A in general. However, in 2017, 34 cases of viral hepatitis A were registered; compared to the previous year, the incidence rate increased by 3.8 times. The incidence rate was 3.54 per 100 thousand population, which is 1.5 times lower than the average of Russia (5.52 per 100 thousand population). Also, routine vaccination against viral hepatitis A in children aged 20 months within the regional calendar of preventive vaccinations continues. With the increase in the number of vaccinated against hepatitis A, the incidence fell to occasional cases

In recent years, hepatocellular carcinoma becomes a serious problem in clinical oncology and hepatology due to the progressive increase in its frequency and the difficulties of early diagnosis. The diagnostics of the initial stage of liver lesions is difficult primarily due to the meager symptomatology and lack of informative general clinical methods of research. This article describes the clinical case of chronic viral hepatitis D, which led to the development of hepatocellular carcinoma in a youngpatient from the Republic of Sakha (Yakutia). The degree of severity of the course of hepatocellular carcinoma of the viral etiology depends on the activity of hepatic tissue damage by hepatitis viruses, the presence of active replication markers, and the signs of liver function decompensation due to the spread of the tumor process. This case dictates the need for mandatory examination of people with chronic viral hepatitis, as a risk group for the development of cirrhosis and liver cancer
